Syllabus Statements and Design
The following tool can be used to develop a course syllabus Introduction to the PDF Syllabus Builder.
The Cal Poly Disability Resource Center recommends that each course syllabus contain a statement reflecting compliance with The Americans with Disabilities Act and Section 504 of the Rehabilitation Act of 1973. Below are some sample statements faculty can consider adding to the syllabus:
- If you have a documented disability and wish to discuss academic accommodations, please contact me as soon as possible.
- If you have a disability for which you are or may be requesting an accommodation, you are encouraged to contact both your instructor and the Disability Resource Center, Building 124, Room 119, at (805) 756-1395, as early as possible in the term.
- Students with disabilities are encouraged to contact the instructor during office hours to discuss their disability related needs. Use of Disability Resource Center services, including testing accommodations, requires prior authorization by the DRC and compliance with approved procedures.
- It is University policy to provide, on a flexible and individualized basis, reasonable accommodations to students who have disabilities that may affect their ability to participate in course activities or to meet course requirements. Students with disabilities are encouraged to contact their instructor to discuss their individual needs for accommodations.
