COVID-19 Test Exemption Request

To request a disability* accommodation for an exemption from Cal Poly's ongoing testing program, please complete the DRC's "Online Student Application" (NOTE: There will be a place to upload disability documentation, if you have it).

Regarding the "Online Student Application" - if you are unsure if you have a disability, or do not know what type of disability, select "other" and write a note about why you should be exempted from testing.  Feel free to skip any questions that do not apply to you, or write, "I don't know" or "Not Applicable".

After you have completed the Online Student Application, we will contact you to schedule a meeting with an Access Specialist.

During the meeting, the Access Specialist will review your Student Application and any documentation you provided, discuss your needs, and let you know if you qualify for an accommodation plan.  If approved, the accommodation plan may include alternative options for testing or a testing exemption.

*Disability is defined as a person with a physical or mental impairment that substantially limits one or more major life activities, or major bodily functions, and the person has a history or record of such an impairment, or is perceived by others as having such an impairment.
