2009 Honorarium Award
For 21 years, Cal Poly's Students with Disabilities Advisory Committee (SDAC) has granted Honorarium awards to students with disabilities. This year the committee will select multiple students to receive SDAC Honoraria of $500 each.
To apply, please carefully complete the application (pdf format/ word format). Finalists will be selected for an interview based on information provided in the application and personal statement. In your personal statement, be sure to address each of the award criteria indicated, and include any other information that may be helpful to the committee in making a decision.
Please return your completed application to the Disability Resource Center, Building 124 - Room 119, by 4:30pm on Friday, November 13, 2009 (late applications will not be accepted).
Applicants will be notified via email no later than mid-January about whether or not they have been selected as a finalist. Finalists will meet with the selection committee for a personal interview in early February.
