The Disability Resource Center welcomes Cal Poly students who are interested in receiving services and accommodations for their disabilities. In order to process requests in a timely manner and to assist you in transitioning to our campus, we request that you do the following:

Step 1:

Complete and return the Student Application for Services as soon as possible so that we can begin to process your request. Applications can be mailed, faxed, or hand delivered to our office.

Step 2:

Send in current supporting disability documentation. Upon receipt of your documentation, an Access Specialist will review the materials. You will be contacted if additional information is needed to determine eligibility for services.

Step 3:

Attend an intake appointment with an Access Specialist. Our office will contact you to schedule an appointment after all necessary documents are received and reviewed.

Step 4:

Discuss appropriate support services during the intake appointment. The Access Specialist will determine your eligibility and identify the services for which you qualify. Services may include: adaptive computer resources, disability management, alternative media, note-taking, sign language interpreting, test accommodations, or on-campus transportation assistance. Once determined eligible, students receive a VISA (Verified Instructional Services and Accommodations) plan that lists recommended accommodations.

Step 5:

Initiate services for the upcoming quarter by filling out the appropriate service request forms.

To ensure the timeliness of services, it is important to begin the process as soon as possible. Implementation of services may be delayed for students waiting until arrival on campus to begin the intake process.

We look forward to meeting you very soon!
